About Our Community Health Centre |

Community Health Centres (CHCs) are non-profit organizations that provide primary health and health promotion programs for individuals, families and communities. Reflective of the needs of the community we are a part of, we are governed by a community-elected board of directors. We work with individuals, families and communities to strengthen their capacity to take more responsibility for their health and wellbeing, and provide education and advice on helping families access the resources they need from other community agencies.

We understand that the health of individuals and communities is impacted by many factors such as income, social support, education and literacy, employment, social and physical environments, personal health practices and coping skills, among others. These factors, referred to as the Social Determinants of Health, may be out of our control but still affect our ability to access and maintain quality health care for ourselves and our families. Our programs and services work to address these issues by offering primary health care, illness prevention and health promotion, and we use a community development approach to building healthy and supportive environments.

You and members of your family can make an appointment to see one of our Doctors, Nurses, Counsellor/Therapists and Community Dietitian. Our staff will take the time to listen and discuss choices that respond to your health care needs. Additionally, you will have access to our 24-hour on-call emergency telephone service and receive a home visit if necessary. Some of our staff speak second languages and, if needed, we have a limited availability to provide interpretation during your visit. We accept clients who do not have health cards and offer appointments with our staff and limited referrals to other medical specialists.

If you live in our catchment area and would like to register as a new patient, please call us at 416.658.6812 to register for our next New Patient Orientation Session. Unfortunately, we sometimes have to place new patients on to a waiting list for service.

Counselling Services
DPNC Counselling services offer individual short- and long-term counselling on personal issues such as abuse, anxiety, parenting and life transitions. Assessment and on-going counselling services for mental health and addiction are also offered. No walk-in appointments available. For more information or to make an appointment call 416-656-8025.
